When someone goes to a hospital, they will probably be getting intravenous (IV) fluids, usually saltwater (saline). Because the solution will be going directly into the veins and dispersed through the circulation to all parts of the body, it is essential that this intravenous saline be completely sterile, and so the containers are tightly sealed and fully sterilized. Therefore there are no viruses, bacteria, mold, or fungus in the saline.
The saltwater that goes into the implant comes directly from one of these sterile IV saline containers. Therefore it is free from any viruses or bacteria, because it was intended to go into the body. That is the reason that it is not harmful to the patient when the implant eventually leaks, which almost all of them will do someday.
It is worth noting that the saline itself does not expire or go bad. But the containers for it are considered to be safe under storage conditions for only a few years, so each is marked with an expiration date, after which they are discarded unused.
