Why don't more Plastic Surgeons know how to put implants in via the navel?

There are several aspects to the transumbilical breast augmentation ("TUBA") that deter Plastic Surgeons from learning how to do the procedure.
  1. Inertia, the natural resistance we all have against learning a new way to do something for which we already have a comfortable way to do it. I often hear, "I've been putting implants in through my favorite incision for ten years, so why should I learn another way to do it?"
  2. Expense, and a disinclination to devote the time and money in the training and equipment. A proper training course takes at least a full week out of someone's busy practice, with course expenses and loss of income, and on top of that, the equipment is quite expensive.
  3. Endoscopic inability, something that handicaps many surgeons. That is an inability to become comfortable with the use of an endoscope, which requires that the surgeon must be capable of interpreting the magnified video image of an internal operating site while controlling the procedure, without being limited to looking at his/her hands. This is an ability that not all surgeons happen to have, even some skillful ones.
  4. Misconception, where the surgeon, out of simple ignorance about the procedure, criticizes it on grounds that are completely false, sort of like those who used to say that "there will never be a time when people have computers in their homes, because computers are much too large and expensive, and besides, why would anyone want one?".
